SUPPLY & HIRE OF MOBILITY AIDS
The Queensland Health Transition Care Program provides goal oriented, time limited and therapy focused care, to help older people at the conclusion of a hospital stay. The care is undertaken at either a residential care facility or in the home environment.
To assist with the care, often mobility and daily living aid equipment is required during this period. Care of a client, is on average for an eight week period.
