Social Housing psycho-social and tenancy support services for the Mental Health Demonstration Project
The Department of Housing and Public Works is seeking Proposals for Social Housing psycho-social and tenancy support services for the Mental Health Demonstration Project.
Responsibilities include (but not limited to):
Provision of short-term mobile/in-home psycho-social and tenancy support services to participants residing within the catchment areas of the Chermside Housing Service Centre or the Fortitude Valley Housing Service Centre
Regular attendance at case coordination/conferencing meetings with other service and support providers
Collaboration with local cross-agency and sector networks to develop and review tailored integrated whole of human service system case-coordination and case management plans for Project participants
The Project has an eighteen (18) month service delivery period from 1 January 2016 - 30 June 2017.
