Disability and Community Care Service Delivery Framework
This project will develop options for structuring an integrated system for delivering community care and specialist disability services to the under 65 cohort after the transition of responsibilities for Home and Community Care programs in 2012. The overarching objective of this project is to provide evidence that will inform the design of the disability and community care system offering a continuum of service responses to deliver the best outcomes for people with a disability in Queensland and best position the sector to meet present and future challenges.
