  • The Supply, Delivery and Installation service for the hire of medical aid equipment for the Transition Care Programme (TCP) and Palliative Care Outreach Service is an essential service required to assist TCP and Palliative Care Outreach Services to transfer patients’ home from hospital, for situations such as.
    1. End of life care where it is the clients wish to die at home
    2. Providing low-level therapy and support to optimise the clients functioning and independence after a hospital stay              
  • This is an essential service required to assist Transition Care and Palliative Care Outreach Services to transfer clients in hospital that could be treated in a home environment and frees up beds for more suitable clients needing services in an acute hospital setting.
